2. INCLUDE ALL SIGNATURES for team members. Parent or guardian signatures are required if player(s) are under 18 years of age. Teams will not be registered without these signatures.
3. MAIL YOUR COMPLETED ENTRY FORM, you entry fee, and any additional charges to:
TEAM HOOPS, P.O. 281, Charlestown, IN 47111.
4. ENTRY FORM must be mailed (postmarked) no later than May 31st, 2008.
5. Your team captain will receive verbal verification along with information about the tournament, your team's division bracket, a complete set of rules, and other events.
6. Teams rating themselves as TOP GUN (adult), are required to complete a Player Profile Form, and submit it with his/her registration form or when signing-in on event day.)
COST:
A non-refundable entry fee of $100.00 must accompany your entry form. You can pay ON-LINE by credit/debit card, or send cashier's check or money order payable to Bay Area Basketball. DO NOT SEND CASH. Personal checks accepted with approval.

TEAM CHECK-IN:
Teams may check-in on the following dates and times: Saturday, June 7th, 2008 starting at 8:00 am, on the square in Campbellsburg, Indiana. All players from each team are required to attend the check-in at least one hour before your team's first scheduled game. Team captain will be called with first game time no later than 9:00 pm, Thursday, June 5th, 2008.
WEATHER POLICY: Adverse weather conditions and unplayable conditions may result in the following: Games may be delayed or the points required to win a game may be reduced. The tournament format may be changed form double elimination to single elimination or other steps may be taken. In the unlikely event of cancellation of the tournament due to adverse weather conditions, each team shall receive their t-shirts and team registration package. Under no circumstances will a full cash refund of the entry fee be refunded.. The exact amount of the refund is at the sole discretion of TEAM HOOPS.
